UK doctor pay is governed by several separate contracts, varied by country:
- 2016 contract (resident / "junior" doctors in England): 5 Nodal Points covering F1 through ST6+
- 2021 SAS contract: Specialty Doctor (12 points) and Specialist (7 points) — career-grade doctors not in training
- 2003/2004 Consultant contract: 5 pay thresholds, progression by years of service
- BMA Salaried GP model contract: a recommended salary range, with 9 sessions/week being full-time
Each country has its own pay scales — Scotland uses a 10-point system, Wales is transitioning to a new spine system from 1 August 2026, and Northern Ireland uses the legacy 2002 contract. On top of basic pay, the 2016 contract pays: weekend allowance, 37% night enhancement, on-call availability allowance (8% resident / 3% non-resident), and additional hours. Weekend and on-call are pensionable; night and additional hours are not.

Consultants in England progress through 5 thresholds: Threshold 1 (entry, £113,565), Threshold 2a (after 3 yrs, £120,249), Threshold 2b (4 yrs, £123,672), Threshold 3 (8 yrs, £135,645), Threshold 4 (14 yrs, £150,569). Progression is automatic with service, subject to job-planning and appraisal.
How is the Salaried GP pay range different from a fixed scale?
The BMA model contract sets a recommended range (£78,699–£118,759 for full-time 9 sessions/week in England) rather than fixed pay points. Salaries within this range are negotiated locally between the GP and practice based on experience, hours and additional responsibilities.
When do new pay scales get published?
NHS Employers and the BMA usually announce pay awards each year in spring or summer, taking effect from 1 April. The figures here will be updated when 2027/28 awards are confirmed.
